Dear Dr. Patel,

I am writing regarding Mrs. Helen Morris, a 62-year-old patient with a long-standing history of rheumatoid arthritis, who presented with worsening bilateral foot pain.

She reports severe discomfort in the metatarsal joints, accompanied by swelling and stiffness, especially in the mornings. Currently, she can walk for only about ten minutes before pain forces her to rest. She also reports increasing difficulty with gardening, which she previously enjoyed.

On examination, she was found to be using worn footwear with minimal arch support. Her reduced mobility is affecting her independence and places her at risk of falls. She remains on her prescribed analgesics and DMARDs.

I recommend the use of custom orthotic insoles and a structured podiatry-based foot care plan to provide better support, improve walking tolerance, and reduce fall risk. These measures should complement her rheumatology care and enhance her quality of life.
