  • C/O: recurrent pain, swelling LL jaw, difficulty chewing

  • Dx: impacted #38 (lower left wisdom tooth)

  • Hx: non-smoker, no allergies, no systemic illness

  • O/E: inflamed operculum, tenderness

  • Radiograph: impacted 38, pericoronitis

  • Rx so far: analgesics, antibiotics (amoxicillin 500mg tds × 5d)

  • Plan: refer for extraction

Dear Dr. Lee,

I am referring Mr. John Matthews, a 45-year-old gentleman, for the surgical extraction of an impacted lower left third molar.

Mr. Matthews has had recurrent episodes of pain, swelling, and difficulty chewing over the past three months. On examination, the operculum was inflamed and tender. Radiographic findings confirmed an impacted tooth (#38) with associated pericoronitis.

He has no systemic illness, allergies, or significant medical history. Management so far has included a course of antibiotics and analgesics, which provided only temporary relief.

Given the recurrent nature of the problem, I believe surgical removal is indicated.
