Emergency Ophthalmic Services

Dear Doctor,

I am urgently referring Mr. David Green, a 71-year-old man, with a suspected retinal detachment in his right eye.

Mr. Green presented with sudden loss of vision in the right eye over the past 24 hours. He described flashes of light, multiple floaters, and a descending curtain-like shadow.

On examination, his visual acuity in the right eye was counting fingers, compared with 6/12 in the left. Fundoscopy revealed a rhegmatogenous retinal detachment in the superior temporal quadrant of the right eye. Intraocular pressures were normal.

Given the acute presentation and risk of permanent vision loss, I request immediate retinal surgical assessment.

Thank you for your urgent attention.

Yours sincerely,
Optometrist
