Dear Dr. Simmons,

I am referring to Mr. Jonathan Reid, a 61-year-old man with long-standing diabetes, who presented with acute swelling and deformity of his left midfoot.

Over the past fortnight, he has noticed progressive swelling, redness, and warmth. He reports little pain, which is consistent with his established neuropathy. Examination revealed early collapse of the midfoot with a rocker-bottom appearance, but no ulceration. Pedal pulses remain palpable.

Mr. Reid had initially assumed the problem was a minor sprain and had been ambulating without restriction, which had likely aggravated the condition. He is overweight and leads a sedentary lifestyle.

In view of these findings, I am concerned about acute Charcot neuroarthropathy. I recommend urgent specialist assessment with consideration for total contact casting or other offloading measures to prevent further deformity and long-term disability.
