Dear Speech Pathologist,

I am referring Mr. Robert Mason, a 70-year-old man with Parkinson’s disease, for assessment and management of speech difficulties.

He presents with hypokinetic dysarthria, characterized by reduced vocal intensity, monotone delivery, imprecise articulation, and fatigue during extended conversation. These difficulties hinder his ability to communicate effectively with family and peers, often requiring repeated clarification. Clinical examination revealed mild facial masking and decreased oral mobility. Mr. Mason is on stable Levodopa/Carbidopa therapy.

We request a structured therapy program to improve speech clarity and loudness, along with strategies for social communication to maximize participation and confidence. Advice for his caregivers to support home practice is also recommended.
