Dear Dr. Watson,

I am referring Mr. David Moore, a 52-year-old patient, for evaluation of a suspicious oral lesion.

He presented with a persistent white patch on his inner cheek, which has been present for two months. The lesion measures approximately 2 × 2 cm and is non-scrapable. It is currently asymptomatic.

Mr. Moore has a significant smoking and alcohol history. He has no other relevant medical issues.

Given the risk factors and clinical presentation, I am concerned about a potentially premalignant lesion and request your further evaluation, including biopsy.
