Case Notes:

  • Patient: Miss Chloe Anderson, 8 yrs

  • Referral: From School Nurse to Speech Pathology Department

  • Presenting Concerns:

    • Frequent repetitions of initial sounds (“b-b-ball”)

    • Occasional prolongations (“ssssun”)

    • Word avoidance in class, increasing anxiety

  • Duration: Symptoms present for past 6 months

  • Family Hx: No speech disorders

  • Social: Bright, active child; good comprehension

  • Teacher Observations: Participates less in oral activities, embarrassed during reading

  • Examination: Normal hearing, normal oral anatomy

  • Nurse Impression: Early-onset developmental stuttering

 

Dear Speech Pathologist,

I am referring Miss Chloe Anderson, an eight-year-old schoolgirl, for evaluation of persistent stuttering that is beginning to affect her academic participation and confidence.

Over the past six months, Chloe has exhibited frequent sound repetitions and occasional prolongations. She often avoids words in classroom discussions, and her teacher notes she is reluctant to engage in reading aloud. These difficulties are accompanied by visible anxiety and self-consciousness.

Chloe has normal hearing, oral anatomy, and overall comprehension. There is no family history of speech disorders. Academically, she is otherwise performing well.

I would be grateful if you could undertake a fluency assessment and advise on appropriate therapeutic interventions, both for Chloe and her parents, to support her communication and self-esteem.
