  • Patient: Mr. Stephen Hall, 48 yrs

  • Complaint: Sudden chest pain, breathlessness

  • Hx: Smoker (20 yrs), hypertension

  • GP suspects pneumonia vs. pulmonary embolism

  • Exam: Temp 38.1°C, BP stable, RR elevated

  • Request: Chest X-ray (PA & lateral)

  • No previous chest imaging on file

To: Radiology Department

Dear Radiographer,

Please perform a chest X-ray (PA and lateral) for Mr. Stephen Hall, aged 48, who presented today with acute chest pain and breathlessness.

He is a long-term smoker with hypertension. His temperature is 38.1°C and his respiratory rate is raised. Clinical impression includes pneumonia or possible embolic pathology. No prior chest films are available for comparison.

Your assistance in obtaining urgent imaging will help guide further management.
