Dear Speech Pathologist,

I am referring Mrs. Angela Morris, a 42-year-old primary school teacher, for assessment and management of a chronic voice disorder.

Mrs. Morris reports persistent hoarseness, throat tightness, and vocal fatigue that intensifies throughout her teaching day. She occasionally experiences brief periods of complete voice loss. ENT examination revealed small bilateral vocal nodules and mild laryngeal irritation. Her past medical history includes reflux gastritis, well-controlled on omeprazole. She is a non-smoker and does not use alcohol regularly.

Given the professional demands of her occupation, early intervention is recommended to prevent further strain and deterioration of voice quality. Please provide a comprehensive voice assessment and an individualized therapy program, including exercises, vocal hygiene strategies, and occupational advice. Parental education is not required; however, strategies to support sustainable voice use in the classroom are requested.
