Dear Radiographer,

I am referring Mr. John Peters, a 74-year-old man, for urgent imaging following a fall at home this morning.

Mr. Peters reports severe left hip pain and is unable to bear weight. On examination, the leg is shortened and externally rotated, which raises a strong suspicion of a hip fracture. He has a history of osteoporosis and hypertension, for which he takes calcium supplements and Amlodipine.

Could you please perform an X-ray of the pelvis and left hip (AP and lateral views) to confirm the diagnosis? He is concerned about radiation exposure so that reassurance would be helpful.

Yours sincerely,
Physician
