Dear Periodontist,

Re: Mr. Robert Turner DOB: 15 May 1975

I am writing to request your expertise in the assessment and potential treatment of Mr. Robert Turner, a 54-year-old patient who presented at Happy Smiles Dental Center with concerns of teeth sensitivity, gum bleeding during brushing and flossing, and occasional bad breath. These symptoms have been affecting him for some time.

Upon intraoral examination, I observed mild to moderate gum recession in several teeth, along with visible dental plaque and calculus buildup. No dental caries or visible lesions were noted. Based on his clinical presentation and history, I have diagnosed him with generalized mild to moderate chronic periodontitis.

Mr. Turner, an accountant in good general health, is keen to address these issues and improve his oral health. To initiate treatment, I recommend professional dental cleaning to address the plaque and calculus buildup, as well as oral hygiene education to prevent further issues. Additionally, desensitizing toothpaste may provide relief from teeth sensitivity.

However, considering the extent of his periodontal condition, I believe a comprehensive evaluation and treatment by a periodontist would be beneficial. I kindly request your assessment and consideration of appropriate periodontal treatment options for Mr. Turner.

Please feel free to contact our clinic if you require any additional information or assistance in coordinating his care.

Yours sincerely,

Dentist
