Dear Ms. Mitchell,

Re: Mr. Daniel Lewis, DOB 15.7.1989

I am writing in reference to Mr. Daniel Lewis, a 30-year-old IT consultant, for physical therapy due to his chronic lower back pain, which has been persistent for the past year. Despite the absence of recent trauma, his condition has worsened progressively.

Mr. Lewis presents with no significant medical history, allergies, or substance intake. His family history includes his mother's breast cancer (currently in remission), his father's hypertension, and various musculoskeletal and cardiovascular conditions in his grandparents.

His occupation as an IT consultant involves prolonged sitting and minimal physical activity, which could contribute to his condition. Presently, Mr. Lewis is diagnosed with chronic non-specific lower back pain, and his BMI of 26.2 indicates that he is overweight.

Given the duration and progression of his symptoms, I recommend a physical therapy evaluation and treatment. Lifestyle modifications, including weight management, should also be considered. In the event that his symptoms persist, imaging studies may become necessary for a more comprehensive assessment.

I kindly request your expertise in addressing Mr. Lewis's chronic back pain and assisting him in regaining his comfort and functionality. Please do not hesitate to contact me if you require further information or have specific questions regarding his case.

Thank you for your attention to this matter.

Yours sincerely,

Doctor
