  • Concern: Risk of spreading infection, osteomyelitis

  • Request: Write to GP for urgent referral to diabetic foot clinic and antibiotic therapy.

Dear Dr. Richards,

I am referring to Mr. George Allen, a 67-year-old man with poorly controlled diabetes, who presented with an infected ulcer on his right great toe.

The ulcer has been present for ten days and now measures approximately 1.5 cm. It exhibits purulent discharge, surrounding erythema, warmth, and a foul odour. Sensation is markedly reduced due to established neuropathy, and his dorsalis pedis pulse is diminished. Mr. Allen reports pain, particularly at night, and increasing difficulty wearing shoes.

He has been applying saline dressings and using OTC analgesics without improvement. His glycaemic control remains poor.

Given the risk of spreading infection and possible osteomyelitis, I recommend urgent referral to a diabetic foot clinic and initiation of systemic antibiotic therapy. Early intervention may prevent hospitalization or amputation.
