To: Dr. Harris

Dear Dr. Harris,

I am writing regarding Mr. Alan Thompson, a 72-year-old patient with a known history of peripheral vascular disease, who presented with worsening claudication symptoms.

He reports cramping calf pain after walking approximately 100 metres, which subsides with rest. These symptoms have gradually progressed over the past six months. Examination revealed cold feet, loss of hair on the lower legs, and weak pedal pulses, suggesting impaired circulation.

Mr. Thompson is currently on aspirin and statin therapy. He quit smoking five years ago but remains largely inactive. He expresses concern about his declining walking tolerance and potential loss of independence.

In view of these developments, I recommend referral for vascular assessment, including Doppler or ankle-brachial index testing, to determine the severity of arterial compromise. Additionally, an exercise-based rehabilitation program may help improve his walking distance and functional mobility.

I would appreciate your assessment and further management.

Yours sincerely,
