I am referring Mr. David Chen, a 52-year-old patient with newly diagnosed Type 2 Diabetes, for comprehensive dietary education and counselling.

Mr. Chen was diagnosed three months ago and commenced on Metformin 500 mg twice daily. His most recent HbA1c remains elevated at 8.9%. Despite initial advice, his dietary habits have not improved. He continues to consume large portions of rice, fried foods, and sugary snacks, with irregular meal timings.

He works in the IT sector and has limited opportunities for physical activity. While he understands the importance of managing his diabetes, his knowledge of carbohydrate management and portion control remains inadequate.

I would appreciate your assistance in providing structured dietary education, including carbohydrate counting, meal planning, and culturally appropriate modifications to his traditional diet. This should help him achieve better glycaemic control and reduce the risk of complications.
