I am writing to inform you that Mrs. Evelyn Martinez will be discharged on 10 August 2023 from Meadowbrook Regional Hospital after receiving treatment for an acute exacerbation of chronic obstructive pulmonary disease (COPD).

Mrs. Martinez was admitted on 29 July 2023 due to increased dyspnea, productive cough, wheezing, and low oxygen saturation. During her stay, we provided oxygen therapy via nasal cannula and managed her respiratory symptoms. I am pleased to report that she has shown mild improvement, and her oxygen saturation has stabilized with minimal oxygen requirement. She is now able to ambulate short distances with assistance.
