Dear Oral Surgeon,

Re: Mr. John Anderson DOB: 15 July 1975

I am writing to request an urgent dental extraction for Mr. John Anderson, a 53-year-old patient who presented at Sparkling Smiles Dental Clinic with a severe toothache and facial swelling in the lower right jaw, consistent with an acute dental abscess related to tooth #30.

Mr. Anderson, an accountant in good general health, has been experiencing excruciating pain and facial swelling for the past three days. His symptoms are localized to the lower right jaw and are exacerbated by biting and chewing. Visual inspection revealed visible facial swelling and tenderness over the right lower jaw, and intraoral examination indicated severe caries and decay in tooth #30 with associated gum inflammation.

Given the clinical presentation and potential risk of systemic spread of infection, I have initiated immediate antibiotic therapy for infection control. However, the definitive treatment for Mr. Anderson's condition necessitates an urgent dental extraction of the affected tooth.

I kindly request your prompt attention to this matter. Mr. Anderson is in significant pain, and timely intervention is essential to alleviate his suffering and prevent further complications. Please do not hesitate to contact our clinic if you require any additional information or assistance.

Yours sincerely,

Dentist
