Dear Ms. Wilson,

Re: Precautions for Mr. Daniel Rodriguez's X-ray

I hope this letter finds you well. I am writing to provide you with important information regarding the scheduled X-ray for Mr. Daniel Rodriguez, which you will be conducting tomorrow.

Mr. Rodriguez, born on 12th March 1955, is a 64-year-old male residing at 789 Pine Avenue, Lakeside. The X-ray has been requested by Dr. Maria Sanchez, our orthopedic surgeon, due to a suspected rotator cuff injury. Mr. Rodriguez's medical history includes hypertension, and he has a known allergy to penicillin. He recently experienced a fall while gardening, which led to the necessity of the X-ray.

During today's appointment at 2:20 pm, Mr. Rodriguez expressed concern and was accompanied by his daughter, Ms. Sophia Rodriguez. Ms. Rodriguez is keen to be present during the X-ray due to her father's anxiety. Please ensure that she wears a lead apron during the exposure.

At 2:35 pm, our Chief Radiographer, Ms. Jennifer Clark, was alerted to the situation. Mr. Rodriguez prefers Ms. Rodriguez to remain in the room with him during the procedure. Additionally, Ms. Rodriguez should stand behind the screen during the exposure. We also have another staff member available to assist if needed.

Regrettably, at 2:45 pm, an urgent request from the Intensive Care Unit (ICU) required our immediate assistance with a critically ill patient, resulting in a delay of approximately 1 hour for Mr. Rodriguez's X-ray.

When notifying Mr. Rodriguez and Ms. Rodriguez of the delay at 2:50 pm, Mr. Rodriguez expressed understanding but remains anxious. There were no security concerns reported. Ms. Rodriguez suggested rescheduling the X-ray for tomorrow afternoon, and I recommend that you take this into consideration.

Please ensure that you communicate Mr. Rodriguez's anxiety, allow Ms. Rodriguez to be present in the room for his comfort, and provide the necessary reassurance during the procedure tomorrow.

Thank you for your understanding and cooperation.

Sincerely,

Radiographer
