Dear Speech Pathologist,

I am referring Master Liam Edwards, a five-year-old boy with a history of repaired cleft palate, for assessment and therapy to address persistent speech difficulties.

Despite undergoing surgical repair at the age of two, Liam continues to exhibit hypernasal speech and frequent articulation errors, particularly affecting consonants such as /s/, /t/, /k/, and /r/. His preschool teacher reports that these difficulties impact peer communication and social participation, leading to frustration and occasional withdrawal. ENT evaluation indicates residual velopharyngeal dysfunction.

We request a comprehensive speech assessment to identify targets for intervention, including articulation and resonance therapy. Parental guidance is requested for home practice to support consistency and reinforce therapy goals. Early intervention is crucial to enhance intelligibility and social confidence in the preschool environment.
