  • Patient: Master Ryan Scott, 12 yrs, student

  • Referral source: Emergency Department

  • Incident: Bicycle accident yesterday afternoon; fell onto the pavement without a helmet

  • Symptoms: Brief loss of consciousness (~2 min), persistent headache, dizziness, vomiting twice since the injury

  • Exam findings: GCS 14/15, drowsy but responsive, pupils equal and reactive, mild scalp tenderness in the right temporal region, no external lacerations

  • Medical history: No epilepsy, no bleeding disorders, no past head injuries

  • Family history: Unremarkable

  • Social history: Active schoolchild, lives with parents, no medication use

  • Request: Non-contrast CT head to rule out intracranial bleed or skull fracture

Dear Dr. Cooper,

I am writing to provide the CT head findings for Master Ryan Scott, a 12-year-old boy who sustained a head injury after a bicycle fall without a helmet.

Non-contrast CT demonstrates a small right temporal extradural hematoma measuring 1.2 cm in thickness, causing minimal midline shift. No skull fracture is identified. The ventricles and basal cisterns are preserved, and there is no radiological evidence of diffuse axonal injury.

Clinically, Ryan presented with headache, dizziness, vomiting, and a brief period of unconsciousness. His GCS is currently 14/15, and pupils remain equal and reactive. He has no significant past medical or family history.

The imaging findings indicate a localized extradural hematoma. Urgent neurosurgical assessment and close monitoring are strongly recommended.
