Dear Dr. Khan,

I am writing regarding Mr. Michael Adams, a 64-year-old patient who has been prescribed Clopidogrel 75mg daily following recent stent insertion.

Mr. Adams has also been on Omeprazole 20mg daily for gastroesophageal reflux disease for the past two years. As you know, Omeprazole can interfere with the antiplatelet action of Clopidogrel, potentially reducing its effectiveness.

Although the patient is currently symptom-free and adherent to his medications, I am concerned about the potential interaction. I have advised him not to discontinue any treatment until reviewed by you.

I would recommend considering switching him to Pantoprazole or an alternative PPI with a lower risk of interaction.

Your guidance on modifying his therapy would be appreciated.
