  • Examination: Mild facial asymmetry, poor oral coordination, dysarthric speech

  • Referral Request: Speech therapy to improve intelligibility, rate, and functional communication

Dear Speech Pathologist,

I am referring Mr. James O’Connor, a 35-year-old patient recovering from traumatic brain injury, for assessment and intervention to improve speech and functional communication.

Following a severe head injury three months ago, Mr. O’Connor exhibits dysarthria with slurred, imprecise speech, reduced speech rate, and fatigue during conversation. Cognition remains intact, but these speech difficulties significantly impact his ability to communicate needs effectively in the rehabilitation setting. Examination revealed mild facial asymmetry and poor oral coordination.

We request therapy focused on articulation, rate control, and strategies to improve functional communication in daily activities. Support and training for caregivers and rehabilitation staff to facilitate communication are also recommended.
