Paediatric Ophthalmology Department
Children’s Hospital

Dear Doctor,

I am referring Emily Johnson, a 4-year-old girl, for evaluation of strabismus and amblyopia.

Emily’s parents report that her left eye has wandered outward since the age of two. She avoids close work and has poor depth perception.

Examination showed reduced acuity of 6/12 in the right eye and 6/24 in the left. A cover test confirmed a constant left exotropia with absent fusion on binocular vision testing. Fundoscopy revealed no structural abnormalities.

Given her young age and the risk of permanent amblyopia, I would appreciate your assessment for treatment options, including occlusion therapy and possible surgical correction of strabismus.

I appreciate your support in Emily’s care.

Yours sincerely,
Optometrist
