I am writing to refer to you Mr. Samuel Brown, a 65-year-old retired accountant residing at 456 Birchwood Road, Birchville, who has been experiencing chronic knee pain for the past two years, which worsens with physical activity. Based on my evaluation, a provisional diagnosis of osteoarthritis of the knees has been made.

Mr. Brown's medical history is unremarkable, with no significant previous ailments. His family history includes his mother, who suffered from breast cancer, and his father, who had type 2 diabetes. His maternal grandmother had Alzheimer's disease, and his paternal grandmother had osteoarthritis. Both of his grandfathers had their respective health concerns.

Currently, Mr. Brown is a non-smoker and consumes alcohol socially, primarily wine with meals. He does not have any known allergies or substance intake.
