    • Cycloplegic refraction: Hyperopia +3.00 RE, +2.75 LE

    • Fundoscopy: Normal OU

  • Current management: Spectacles prescribed 2 months ago, minimal improvement in alignment

Dear Doctor,

I am referring Jacob Reed, a 7-year-old boy, for assessment and management of a right-sided strabismus.

Jacob’s parents have observed an inward turn of his right eye over the past six months. He sometimes reports double vision and struggles with reading, which his schoolteacher has also noted. His concentration in class has been affected, and he tends to avoid reading aloud.

Examination revealed reduced visual acuity in the right eye (6/18) compared to the left (6/9). A cover test demonstrated a constant right esotropia, and stereopsis was absent. Cycloplegic refraction showed moderate hyperopia (+3.00 RE, +2.75 LE). He has been wearing corrective spectacles for two months, but ocular alignment has not improved significantly. Fundoscopy was normal in both eyes.

Given the persistence of deviation and its functional impact, I would appreciate your evaluation to determine whether orthoptic therapy or surgical correction would be appropriate at this stage.
