15 May 2030 Presenting problem: Mr. Mitchell reports that his dental crown (upper right first molar) feels loose and occasionally comes off while eating. He reports discomfort and mild pain when the crown is loose.

Symptoms:

  • Loose dental crown (upper right first molar)

  • Discomfort and mild pain when crown is loose

  • No visible swelling or gum issues

Intraoral Examination:

  • Loose dental crown in upper right first molar area

  • No signs of infection or abscess

  • No other significant dental issues

Diagnosis: Loose dental crown in the upper right first molar area

Initial treatment: Recementation of the dental crown. Referral for evaluation if recurrent issues occur.

Writing Task:

I am writing to request your expertise in the assessment and potential recementation of a dental crown for Mr. James Mitchell, a 64-year-old patient who presented at Sunshine Dental Care. Mr. Mitchell reported issues with a loose dental crown in the upper right first molar area, occasionally causing discomfort and mild pain while eating.

Upon intraoral examination, I confirmed the presence of a loose dental crown with no signs of infection or abscess. No other significant dental issues were noted. Based on his clinical presentation, I have diagnosed him with a loose dental crown in the upper right first molar area.

Mr. Mitchell, a retired individual in good general health, is keen to address this issue promptly to alleviate his discomfort and ensure the stability of the dental crown. To address the concern, I recommend recementation of the dental crown.
