Dear Dr. Howard,

I am writing regarding Mr. Daniel Young, a 29-year-old recreational runner, who presented with pain in his right foot suggestive of a stress fracture.

The pain began three weeks ago, coinciding with a sharp increase in his training mileage. It localizes to the dorsum of the foot and is exacerbated by weight-bearing activity, but settles with rest. Clinical examination revealed focal tenderness over the second metatarsal shaft and mild swelling. He demonstrates an antalgic gait.

Mr. Young attempted rest and icing initially, but resumed training prematurely, which has perpetuated his symptoms. His medical history is clear of systemic conditions.

Given the clinical picture, I suspect a metatarsal stress fracture. I recommend diagnostic imaging (X-ray or MRI) for confirmation and referral to a sports medicine specialist for management and graded return-to-play guidance.
