Dear Dr. Matthews,

I am writing regarding Mr. Daniel Harris, a 63-year-old man with long-standing Type 2 Diabetes, who presented with a non-healing ulcer on his left foot.

The ulcer, located laterally, measures approximately 1 cm and has been present for two weeks. Although there is no pus discharge, there is mild surrounding erythema, raising concern for early infection. Examination revealed peripheral neuropathy with reduced sensation and a faint dorsalis pedis pulse. His skin is generally dry and cracked.

Mr. Harris has been attempting dressings at home but admits inconsistent compliance. His risk factors include poor diabetic control, obesity, and smoking, all of which compromise healing.

Given the risk of progression and infection, I recommend urgent vascular review and referral to a multidisciplinary diabetic foot clinic for comprehensive care.
