Dear Dr. Evans,

I am referring Mr. Daniel Morris, a 39-year-old recreational runner, for further evaluation of chronic left Achilles tendinopathy.

Mr. Morris has reported Achilles tendon pain for the past four months, associated with morning stiffness and tenderness on palpation. He has trialled rest, eccentric loading exercises, and heel lift orthotics. Additionally, he attended six podiatry sessions involving strengthening, stretching, and ultrasound therapy.

Although he has experienced some pain reduction, he remains unable to resume full running activity. Given the persistent nature of his symptoms, I believe further assessment, including imaging and possible advanced interventions, is warranted under your care.

He will continue conservative podiatry management until your review.

Thank you for your assistance with this case.

Yours sincerely,
