Dear Dr. Mitchell,

Re: Referral of Mr. William Turner - Rotator Cuff Tear Rehabilitation

I hope this letter finds you well. I am writing to introduce you to Mr. William Turner, a 50-year-old carpenter who has been referred to our clinic for rehabilitation following a significant rotator cuff tear in his right shoulder.

During his initial assessment, Mr. Turner reported severe pain in his right shoulder, particularly during overhead activities, along with reduced range of motion, muscle weakness, and difficulties with lifting and reaching. Physical examination revealed tenderness, swelling, and limited range of motion in the right shoulder joint, as well as decreased muscle strength in the right upper arm. Positive impingement tests further confirmed our diagnosis.

Based on our evaluation, I have diagnosed Mr. Turner with the need for right rotator cuff tear rehabilitation. I believe that your expertise and experience in physiotherapy will greatly benefit him in his journey toward recovery.

To initiate his rehabilitation, I have developed an individualized treatment plan, which includes strategies for effective pain management, range of motion exercises to improve shoulder mobility, strengthening exercises to restore muscle function, and a gradual return-to-work program tailored to his occupation as a carpenter.

I am confident that your guidance and support will help Mr. Turner regain strength and function in his right shoulder, allowing him to return to his daily activities and work with confidence. Please feel free to reach out to me for any additional information or clarification regarding his condition and treatment plan.

Thank you for your valuable assistance in Mr. Turner's rehabilitation journey.

Warm regards,

Physiotherapist
