Dear Dr. Patel,

I am writing to report MRI lumbar spine findings for your patient, Mr. David Lane, a 45-year-old warehouse worker with chronic low back pain and left-sided sciatica.

MRI sequences reveal a large left paracentral L4–L5 disc herniation causing compression of the left L5 nerve root. There is an associated narrowing of the left lateral recess. The remaining lumbar discs show mild degenerative changes, with no evidence of spinal stenosis. No vertebral fracture or suspicious lesion is identified.

Clinically, Mr. Lane reports persistent back pain radiating to his left leg, along with sensory disturbance and reduced reflexes. His occupational demands and smoking may contribute to ongoing spinal stress.

The imaging confirms a left L4–L5 disc herniation with nerve root compression, correlating with his symptoms. Referral to spinal services for further management is advised.
