To: Dietitian, Bariatric Surgery Follow-Up Clinic

Dear Dietitian,

I am referring Mrs. Eleanor White, a 41-year-old woman who underwent gastric bypass surgery six weeks ago, for ongoing nutritional support.

Mrs. White had a pre-surgery BMI of 42 and a history of hypertension and sleep apnoea. Since surgery, she has lost 9 kg and now weighs 91 kg. However, she reports difficulty tolerating solid foods, frequent nausea after meals, and inconsistency with her vitamin supplementation.

She remains motivated and adheres to her follow-up care. Nevertheless, she requires specialist dietetic guidance on protein intake, vitamin and mineral supplementation, and appropriate progression of her diet to optimize recovery and weight management.

I would appreciate your input in developing a structured post-bariatric diet plan and providing ongoing education to support her long-term health.

Yours sincerely,
