Dear Specialist,

I am referring Mrs. Anita Shah, a 64-year-old woman, for evaluation and management of suspected advanced glaucoma.

Mrs. Shah has been experiencing progressive peripheral vision loss. She has a family history of glaucoma, which further raises concern.

Examination findings included intraocular pressures of 28 mmHg (right) and 29 mmHg (left). Cup-to-disc ratios were enlarged bilaterally (0.7 and 0.75). Visual field testing showed advanced peripheral loss, including an inferior arcuate scotoma. Visual acuity remains preserved at 6/9 in both eyes.

Given these findings, I suspect primary open-angle glaucoma. I would appreciate your assessment for confirmatory diagnostics and initiation of treatment, whether medical or surgical.
