  • Patient: Mr. James O’Connor, 23 yrs, university student

  • Referral source: Emergency Department

  • Presenting issues: Acute onset of right lower abdominal pain ×12 hrs, nausea, anorexia, mild fever (38.2°C)

  • Exam findings: Localized tenderness and rebound at McBurney’s point, mild guarding, no palpable mass

  • Investigations: Bloods – WBC 14,000; CRP elevated

  • Medical history: Fit and well, no previous surgeries, no medication use

  • Family history: Non-contributory

  • Social history: Active, lives in student accommodation, no alcohol, non-smoker

  • Request: Contrast-enhanced CT abdomen/pelvis to confirm appendicitis and exclude complications

Dear Dr. Harris,

I am writing to provide CT abdomen and pelvis findings for your patient, Mr. James O’Connor, a 23-year-old man with acute abdominal pain.

Contrast-enhanced CT demonstrated a dilated appendix measuring 11 mm in diameter with wall thickening and periappendiceal fat stranding. A small appendicolith is noted within the lumen. No perforation, abscess, or free fluid is identified. The remainder of the bowel and abdominal organs appear normal.

Clinically, Mr. O’Connor presented with right lower quadrant pain, nausea, anorexia, and mild fever. His inflammatory markers were raised. He has no relevant past medical history.

The imaging findings confirm acute appendicitis without complications. Surgical review for appendectomy is advised.
