Dear Dr. Patel,
I am referring Mr. John Harris, a 56-year-old gentleman with persistent hypertension, for your cardiology assessment and management.
Mr. Harris was diagnosed with hypertension six months ago and commenced on Amlodipine 5 mg once daily. Despite adherence to his medication and lifestyle advice, his blood pressure remains elevated, with the most recent reading at 165/98 mmHg. He has also reported occasional mild headaches but denies chest pain, palpitations, dizziness, or visual disturbances.
Mr. Harris is overweight, with a BMI of 30, and leads a sedentary lifestyle due to his office-based work. He acknowledges consuming a high-salt diet and minimal fruit and vegetables, despite repeated counselling. His family history is significant for cardiovascular disease, as his father suffered a myocardial infarction at 60 years.
Given his poor response to initial management, I would appreciate your evaluation for secondary causes of hypertension, consideration of additional antihypertensive therapy, and further investigations as necessary.
In the meantime, I will continue to encourage him to increase his physical activity and adopt dietary modifications.
Yours sincerely,
