Presenting problem: Sudden decrease in vision in the left eye

Left eye:

  • Vision loss started yesterday

  • No pain or discomfort

  • No flashes or floaters

  • No history of trauma

  • Currently taking medication for hypertension

Visual acuity (VA): L 6/60, R 6/6

Colour vision (D-15): Normal

External examination:

  • No obvious abnormalities

  • No redness or swelling

  • No signs of injury or inflammation

Slit-lamp examination:

  • Anterior chamber is clear

  • No signs of infection or inflammation

  • No abnormalities in the lens

Intraocular pressure (IOP) (Goldmann):

  • L 16mmHg

  • R 18mmHg

Fundoscopy:

  • Left eye – significant subretinal hemorrhage with macular involvement

  • Right eye – stable AMD, no active bleeding

Diagnosis: Acute macular hemorrhage in the left eye, likely related to AMD

Initial treatment: No immediate intervention. Advised patient to return if symptoms worsen.

I am writing to seek your expertise and assistance in evaluating and managing Mr. James Anderson, a 66-year-old patient who presented to our clinic yesterday with a sudden decrease in vision in his left eye.

Mr. Anderson has a medical history of age-related macular degeneration (AMD), diagnosed in 2016, for which he has been receiving regular check-ups. In 2010, he underwent cataract surgery on his right eye. His general health is relatively stable, with controlled hypertension being the only significant medical condition.

The sudden vision loss in his left eye began yesterday without any associated pain, discomfort, flashes, or floaters. Importantly, there is no history of trauma. His visual acuity in the left eye is reduced to 6/60, while the right eye maintains a visual acuity of 6/6.

Clinical examination of the left eye reveals a significant subretinal hemorrhage with macular involvement. The anterior chamber appears clear, and there are no signs of infection or inflammation. Intraocular pressure measurements are within normal limits.

Given the presentation and findings, my provisional diagnosis is acute macular hemorrhage in the left eye, likely related to AMD. I have not initiated immediate intervention and have advised Mr. Anderson to return if his symptoms worsen.
