Dear Dr. Shaw,

I am writing to refer Mrs. Clara James, a 56-year-old shop assistant, who presented with a painful bunion on her right foot that has been worsening in recent months.

She has been experiencing constant aching pain at the first metatarsophalangeal joint for the past 18 months, with significant aggravation when walking or standing for more than 15 minutes. Despite trialling wider footwear and bunion pads, she reports little relief. Examination revealed a considerable bony prominence with associated redness and tenderness, and the second toe is beginning to overlap.

Her general health is stable apart from controlled hypertension. However, her condition is impacting her work, as she struggles with prolonged standing and shoe fitting.

In view of the progression and functional limitation, I recommend referral to an orthopedic surgeon for further assessment and consideration of surgical correction.

Thank you for your assistance in managing her case.

Yours sincerely,
