Dear Dr. Harris,

I am referring Mr. Robert Collins, a 58-year-old office manager, for further assessment of his right shoulder.

Mr. Collins sustained a partial rotator cuff tear three weeks ago while lifting boxes at home. Since then, he has experienced persistent shoulder pain, especially at night, along with reduced ability to perform overhead movements. His medical history includes controlled hypertension and mild osteoarthritis of the knees.

He was prescribed pain relief medication by his GP and referred to physiotherapy. In our sessions, we have focused on ultrasound therapy, posture advice, and gentle mobility exercises. Mr. Collins has been compliant with his home program, reporting slight pain improvement, but functional movement remains significantly limited.

Given his restricted progress, I believe an orthopedic review is warranted to consider further management options.
