Dear Specialist,

I am referring Ms. Laura Bennett, a 35-year-old woman with suspected progressive keratoconus, for further management.

Ms. Bennett has been experiencing fluctuating vision for two years, with frequent changes in prescription. She reports ghosting of images and an increase in glare, particularly at night.

Her visual acuity measured 6/12 in the right eye and 6/24 in the left. Refraction showed irregular astigmatism, and corneal topography demonstrated inferior steepening bilaterally, consistent with keratoconus. Examination also revealed thinning with Vogt’s striae. She has tried rigid contact lenses but can only tolerate them for short periods, leaving her functionally impaired.

Given the progression and poor tolerance to conservative management, I would appreciate your assessment for corneal cross-linking to halt disease progression and discussion of surgical options such as keratoplasty if indicated.
