I am writing to recommend Mr. John Martinez, a 55-year-old accountant, for additional testing and advice about his persistent lower back pain. His illness has not improved after six months of treatment with over-the-counter painkillers and physical therapy.

 

Mr. Martinez's medical history includes an appendectomy in 1998, gastroenteritis in 2005, and a diagnosis of mild hypertension in 2010, which he has managed with lifestyle changes. His family history includes his mother's survival of breast cancer and his father's type 2 diabetes. Additionally, his maternal grandmother had Alzheimer's disease, and his paternal grandmother had osteoporosis.

 

Presently, Mr. Martinez complains of chronic lower back pain, with no history of trauma or recent heavy lifting. Given the persistent nature of his condition, I suspect further evaluation is necessary. Therefore, I recommend that he be seen by an orthopedic specialist for a thorough assessment.
