Dear Dr. Harris,

I am referring Mr. Andrew Clarke, a 45-year-old school teacher, for further management of chronic plantar fasciitis.

Mr. Clarke has experienced right heel pain for the past five months, worse in the morning and after periods of rest. Conservative podiatric treatment, including orthotics, stretching exercises, ultrasound therapy, and taping, has resulted in partial improvement. He has also received lifestyle advice and NSAID therapy from his GP.

Despite these interventions, his pain persists, particularly with prolonged walking, limiting his daily activities.

Given the chronicity of symptoms and only partial response to therapy, I believe corticosteroid injection under your care may be beneficial. He will continue supportive podiatry sessions alongside any further treatment you recommend.

Thank you for your assistance in this case.

Yours sincerely,
