  • Social history: Lives alone, non-smoker, occasional wine

  • Request: X-ray right wrist to rule out fracture and assess displacement

Dear Dr. Miller,

I am writing to provide the X-ray findings for your patient, Mrs. Helen Brooks, a 67-year-old woman who sustained a wrist injury following a fall.

Plain radiographs of the right wrist reveal a comminuted fracture of the distal radius with dorsal angulation and mild displacement. There is also an associated fracture of the ulnar styloid. Carpal bones remain aligned, and no dislocation is identified. Periarticular soft tissue swelling is evident.

Clinically, Mrs. Brooks presented with wrist deformity, pain, and functional limitation after a fall onto her outstretched hand. Her radial pulse and finger movement remain intact. She has a known history of osteoporosis, making her prone to fragility fractures.

The imaging confirms a displaced distal radius fracture with an associated ulnar styloid fracture. Orthopedic intervention is recommended to restore alignment and function.
