To: Dr. Patel

Dear Dr. Patel,

I am writing to refer Mrs. Catherine Lewis, a 48-year-old retail worker, who presented with a painful and progressive bunion on her right foot.

She has experienced increasing discomfort for the past 18 months. Despite using wider footwear and bunion pads, her symptoms persist. Examination revealed a marked hallux valgus deformity with a prominent medial eminence and overlapping of the adjacent second toe. The area is tender to palpation, and her mobility is increasingly restricted.

Mrs. Lewis’s occupation requires prolonged standing, which exacerbates her pain and limits her ability to carry out her role effectively. Her medical history is otherwise unremarkable.

Given the severity of her symptoms and failure of conservative management, I believe surgical intervention may be warranted. I therefore request your evaluation for a possible bunionectomy or corrective procedure.

Thank you for considering this referral.

Yours sincerely,
