Dear Dietitian,
I am referring Mr. Paul Edwards, a 49-year-old man with Type 2 Diabetes, for dietary education and structured lifestyle counselling.
Mr. Edwards was diagnosed with diabetes one year ago and commenced on Metformin 500 mg twice daily. Despite this, his most recent HbA1c remains poorly controlled at 9.2%. He reports difficulty adhering to dietary recommendations and admits to frequently consuming sugary foods and snacks. He also struggles with portion control and often skips meals due to his busy work schedule.
His BMI is 29, placing him in the overweight category. He does not engage in regular physical activity, although he has expressed a willingness to improve this. He has no diabetes-related complications to date, but given his poor glycaemic control, he is at increased risk of developing long-term complications.
I believe he would greatly benefit from your guidance on carbohydrate management, portion sizes, and meal planning that fits his lifestyle. Additionally, structured education may help him adopt sustainable dietary habits and reduce reliance on processed foods.
I would appreciate your support in providing him with practical strategies to improve glycaemic control and reduce future cardiovascular risk.
Kind regards,
