This letter is in reference to Mr. David Mitchell, a 50-year-old office manager, who has been experiencing chronic lower back pain for the past six months. The pain has progressively worsened, despite the absence of recent trauma.

Mr. Mitchell's medical history is unremarkable, and he reports no allergies or substance intake. However, his family history includes his mother's battle with breast cancer (currently a survivor) and his father's diagnosis of type 2 diabetes. His grandparents have also experienced various health conditions, including Alzheimer's disease, osteoporosis, and heart disease.

As an office manager, Mr. Mitchell's work primarily involves prolonged periods of sitting, which may contribute to his condition. His BMI of 27.2 indicates that he is overweight.

Presently, Mr. Mitchell is provisionally diagnosed with chronic non-specific lower back pain. I believe that physical therapy, along with lifestyle modifications and ergonomic assessments at work, would be beneficial in managing his symptoms and improving his overall quality of life.
