Case Notes:

  • Patient: Mrs. Rita Fernandez, 56 yrs

  • Complaint: Chronic low back pain, worsening in the last 3 months

  • Hx: Degenerative disc disease, sedentary occupation

  • Examination: Restricted lumbar movement, tenderness over L4-L5

  • Medications: Ibuprofen PRN

  • GP requests Lumbar spine X-ray ? rule out progression of spondylosis

  • No known allergies, no recent trauma

To: Radiology Department

Dear Radiographer,

Please perform a lumbar spine X-ray for Mrs. Rita Fernandez, a 56-year-old woman with long-standing degenerative disc disease.

She presents with worsening lower back pain over the past three months. Examination revealed restricted lumbar movement and localized tenderness at L4-L5. She has no history of trauma, recent injury, or systemic illness.

Kindly obtain AP and lateral views of the lumbar spine to assess the progression of spondylotic changes.
