Dear Dr. Brown,

I am referring Mrs. Linda Carter, a 50-year-old patient, for management of advanced periodontal disease.

Mrs. Carter presented with gum bleeding, halitosis, and loose teeth. Examination revealed generalized deep pockets and mobility of multiple teeth. Radiographs showed extensive alveolar bone loss.

She has a history of controlled hypertension but is otherwise healthy. Initial scaling was performed, but due to the severity of the condition, specialist intervention is necessary.

I would appreciate your evaluation and ongoing management of this case.
