Dear Dr. Adams,

Re: Referral of Mr. Samuel Roberts - Stuttering and Speech Fluency Disorder

I trust this letter finds you well. I am writing to introduce you to Mr. Samuel Roberts, a new patient at our clinic who has been referred to us for the assessment and management of speech fluency issues, characterized by difficulty with word articulation and prolonged pauses while speaking.

Mr. Roberts, a 47-year-old bank manager, is in generally good overall health and does not have any significant medical conditions. Importantly, he has no previous history of speech fluency issues or related concerns. However, he recently experienced a sudden onset of difficulties in speech fluency, which include difficulty articulating certain consonants and prolonged pauses during speech. There are no signs of physical or neurological issues or craniofacial abnormalities. Despite this, the condition has caused him frustration and embarrassment due to communication difficulties.

Based on our assessment, we have diagnosed Mr. Roberts with Stuttering and Speech Fluency Disorder. We have initiated speech therapy sessions to address his speech fluency issues, improve articulation, and develop strategies for more fluent communication.

I kindly request your expertise in overseeing Mr. Roberts' condition and further assessment, particularly if there is a need for neurological evaluation or if any underlying factors are contributing to his speech fluency issues. If you believe that any adjustments to the treatment plan or additional diagnostic procedures are warranted, please do not hesitate to contact me. Mr. Roberts will return for a follow-up appointment in six weeks to assess his progress.

Thank you for your collaboration in ensuring the best care for our patients. If you require any additional information or have any questions concerning this referral, please feel free to contact me.

Yours faithfully,

Speech Pathologist
