Dear Dr. Roberts,

Re: Mrs. Sarah Johnson, DOB 15th June 1980.

I am writing to provide an update on your patient, Mrs. Sarah Johnson, who came to our clinic for fertility counseling. Following our consultation, here is the plan we have discussed:

Mrs. Johnson has been experiencing difficulty conceiving for the past 18 months, which is understandably causing her frustration and stress. She has irregular menstrual cycles and a history of Polycystic Ovary Syndrome (PCOS) diagnosed in 2017. No previous pregnancies have been reported.

In terms of her anthropometric and biochemical data, her BMI is 25.7, and her hormonal profile shows elevated LH levels, consistent with her PCOS diagnosis. Her TSH and prolactin levels are within the normal range.

I have advised Mrs. Johnson to make some lifestyle modifications, including stress management techniques and regular exercise. We have discussed a balanced diet tailored to her PCOS condition. She will continue her Metformin medication.

Our immediate plan involves scheduling a follow-up appointment in 3 months to assess progress. Additionally, I am referring her to a fertility specialist for a comprehensive evaluation.

Please feel free to reach out if you require any further information or have specific questions regarding Mrs. Johnson's case.

Yours sincerely,

Dietitian
