Dear Speech Pathologist,

I am referring Mrs. Angela Morris, a 42-year-old schoolteacher, for voice assessment and therapy.

She presents with a hoarse, strained voice, persisting for the last five months and worsening with extended speaking. ENT evaluation revealed small bilateral vocal nodules with mild laryngeal irritation. She has a history of reflux, managed with omeprazole, but no smoking or alcohol use.

Please provide targeted voice therapy and education on vocal hygiene strategies suitable for her profession.

Yours sincerely,
